Volunteer Time and Donation Forms
These are Oregon Parks and Recreation Dept. Forms, but we can use them for all grant submissions. The important thing is that we collect a record of giving so that we can use it in our grants. These are Word Documents that can be printed out.
Submit completed forms to City Hall or one of the Steering Committee members.
